Refining the Definition of "Low Risk" in PAH — REVEAL 2.0 ≤4

- Proposed refined definition: "Risk of death <5% at 3 years AND risk of clinical worsening <10% at 1 year" (vs the current 1-year mortality-only definition)
- Clinical worsening composite: Lung transplant, PAH hospitalization, initiation of long-term O₂, ≥15% drop in 6MWD with worse NYHA, or addition of a new PAH medication
- Key findings:
- Patients with REVEAL 2.0 score ≤4 met the refined low-risk definition: 3-year mortality 2.6%, 1-year clinical worsening 6.4%
- Patients with score 5-6 had worse hemodynamics and presentation, did NOT meet the refined definition
- Independent predictors of refined low risk: idiopathic PAH, longer 6MWD, lower NT-proBNP, NYHA I-II, lower heart rate, absence of prior AF
- Why it matters (clinical takeaway):
- Tightens the threshold for declaring a PAH patient "at goal." REVEAL 2.0 ≤4 is the new low-risk benchmark — anything 5-6 should prompt consideration of treatment escalation.
- First risk definition to embed morbidity at 1 year + mortality at 3 years rather than 1-year mortality alone — better reflects what patients care about.
- Use alongside the ESC/ERS 4-strata risk tool. The refined REVEAL ≤4 may identify patients who can safely de-escalate or maintain on a 2-drug regimen vs needing triple therapy or prostacyclin escalation.
- Caveats / limitations: Pooled analysis of RCT populations (younger, less comorbid than real-world). Validation in registries needed.
